Dr. Cassidy and Dr. Guerin are participating with Medicare, therefore, only the 20% co- payment is required at the time of service, unless your supplement is a “Medigap” policy that Medicare transfers the claims to, which pays us directly.

• Gulf Coast Healthcare Sarasota

• Web TPA
• Medicare
• Freedom (Medicare/Medicaid effective after January 1, 2014)

We also will work with most other insurance companies, providing that you have out of network benefits. Please talk to one of our insurance specialists.

If we participate with your insurance company, you will be responsible at the time of service for any co-payment, co-insurance, or deductible amount.

If we do not participate with your insurance company, full payment is expected at the time of service. For your convenience we accept cash, checks or credit/debit cards. As a courtesy we will file all physician, hospital and surgical charges.
